The artwork makes me think of physical pain, and a physical ailment or enemy "buried" within. But it works for a psychological or emotional conflict as well.

Yes, this works on a physical and emotional level and the last line is very powerful.

makes me think of someone's past catching up with them no matter the distance that they put between it

it made me think of anger, like fueding groups of people upon sight of eachother their anger unleashes and a usually rational person become irrational due to pride.
